Components of the preparation
The main active substance of the ointment is sulfatiazolesilver, auxiliary:
- water;
- methylhydroxybenzoate;
- liquid paraffin;
- Vaseline;
- propylhydroxybenzoate;
- cetostearyl alcohol;
- glycerol;
- potassium hydrophosphate.
Pharmacological properties and mechanism of action
Argosulfan is an antimicrobial agent for topical application. Silver salt( sulfatiazole) in its composition promotes the inhibition of dihydropteroate synthetase, which leads to a halt in the synthesis of dihydrofolic acid and its active metabolite, tetrahydrofolic acid. The latter participates in the synthesis of pyrimidines of the microbe cell and the synthesis of purines.
Silver ions in the composition of the ointment stop the division and growth of bacteria due to the fact that they bind to the DNA of the microbial cells. Also, 
silver ions reduce the sensitizing properties of sulfonamide.
The minimum solubility of silver salt helps maintain a stable level of drug concentration in the wound for a long time. Resorption of the drug is minimal, no toxic effect was observed.
After applying the ointment, only a minimal amount of silver salt is found in the bloodstream, which is then acetylated in the liver.
In urine, the drug accumulates in the form of inactive, partially unchanged metabolites.
Indications for use
Sulfanilamide( sulfatiazole silver) in the preparation has bacteriostatic, antimicrobial effect, is harmful to gram-negative and gram-positive bacteria.
Cream Agrosulphan heals wounds of various etiology: purulent, burn, trophic, mechanical, etc. Prevents their infection, eliminates burning, pain, significantly reduces the preparation of a wound for skin transplantation, and in some cases makes transplantation unnecessary.
The product is used for:
- cuts, abrasions;
- frostbite;
- purulent wounds;Pressure sores;
- trophic ulcers of the lower leg of various origin( venous insufficiency, angiopathy, erysipelas, diabetes mellitus), infected dermatitis( obliterating endarteritis).
Application in phlebology
In phlebology, Agrosulfan is successfully used for trophic ulcers. Trophic ulcers are open wounds localized on the feet and legs. The reason for their appearance is a disease associated with varicose veins, postthrombophlebitic or arterial disease.
A characteristic feature of trophic ulcers is a very long healing period - at least six weeks, as well as spread over the leg in the absence of adequate treatment.
If an additional examination of the wound reveals that it is infected with fungi or non-spore forming anaerobic organisms, it is necessary to act quickly, since there is a risk of infection of the blood and the development of sepsis.
Among its undeniable advantages is the lack of the need to use an additional antibiotic in the treatment of ulcers. The drug is a broad-based medicine that destroys virtually all types of gram-negative and gram-positive pathogens.
Dosage and therapy regimen of
Agrosulfan is applied as a direct application to the wound surface, followed by the application of an occlusive( airtight) dressing.
First, the wound must be treated with a disinfectant, in accordance with the conditions of sterility. For these purposes, a one-percent solution of iodopyrone or hydrogen peroxide can be used.
When processing it is important to capture not only the surface of the ulcer, but also the surrounding skin. After this, you can start applying the drug.
The cream is applied in a thin layer( 2-3 mm) two or three times a day. The entire wound should be covered with a layer of ointment.
How to make an occlusive dressing: 
- the patient should be in bed;
- spirally apply a gauze bandage to the leg, make sure that it does not form wrinkles;
- over the bandage again worn ointment and on top of everything again tied with a sterile bandage;
- do this operation four or five times.
Duration of the procedure - until complete recovery. It is advisable that before applying the bandage, the patient spent a day in bed to remove swelling of the legs.
When bandages are changed, it is often found that exudate( fluid) is formed on the wound. In this case, the ulcer is washed with a 3% aqueous solution of boric acid, a 0.1% aqueous solution of chlorhexidine or another antiseptic.
The maximum duration of treatment is two months.
Side effects of
Typically, Agrosulfan is well tolerated by patients. But sometimes, after applying the drug, the subjects observed:
- allergic reactions of a large nature( with hypersensitivity to the substances contained in the composition of the ointment);
- local allergic reactions( itching, burning, redness of the skin);
- desquamatous dermatitis;
- leukopenia.
The last two items of the above list refer to cases of prolonged use of the ointment.
Contraindications and special instructions
Agrosulfan is contraindicated in the following diseases and conditions:
- increased individual intolerance to the components of the cream, especially sulfonamide;
- breast-feeding;
- congenital insufficiency of glucose-6-phosphate dehydrogenase;
- children under the age of two months( risk of developing nuclear jaundice);
- premature babies( may develop nuclear jaundice).
Patients with renal or hepatic insufficiency should be monitored for serum sulfatiazole.
For those patients who are in shock or have extensive burns, the drug is prescribed with caution, since in these conditions the doctor does not have the opportunity to collect a complete allergic anamnesis.
The use of the drug does not affect the exercise, requiring rapidity of psychomotor reactions and increased concentration of attention( for example, driving a car).
Pregnant women and breastfeeding mothers can use Agrosulfan only if the doctor finds that the potential benefit for a woman is greater than the likely risk to the fetus or the baby.
